Choosing at home in the US or Canada trades a lower price for things that only being local can give you. None of these is a universal win, but together they are real.

Surgeon-led. Onshore clinics are more likely to have the surgeon performing or directly overseeing the work, and you can meet them in person before you commit.
In-person follow-up. The wash days, the check-ins, and any first-week concerns happen in the room, not on a screen or a return flight.
Onshore recourse. A contract dispute or a correction has a familiar, local path to redress - the protection that distance quietly erodes.
No travel recovery. You recover where you live, sleep in your own bed, and skip the flights, hotels, time-zone shifts, and a multi-week stay.

When local wins vs when it’s just more expensive

The worth of local is personal, not absolute. The honest trade, with no pressure either way.

Local wins when in-person follow-up, a surgeon you can sit with, and straightforward onshore recourse matter more to you than the price gap - for example, if your recovery anxiety would be higher in an unfamiliar place, or if a correction being easy is important.
Local is just more expensive when your always-on-hand follow-up options are strong enough (or your budget is tight enough) that the difference mainly buys comfort rather than capability.
Both are legitimate. The calm rule is the same as every route: judge the clinic, the contract, and the quoted aftercare plan on their own merits, and weigh the four axes by your own priorities rather than a price reflex in either direction.
